In reviewing the ABC’s MMA National Identification card application form (Revised 06/05/2009), number 1 of the Terms and Conditions states: Birth name must be used in top name field. If there is a different first or last name place either or both in Also known as line. However, there is no “Also known as” line in the name fields of the application.
Please advise if there is a revised application that addresses this issue. Thanks!
